Welcome to the Clear Water Pain Clinic. Pain interferes with your normal daily life and you might have already found yourself isolated and depressed. Pain is your personal experience and you are the expert of your own pain. So, your pain is what you describe to us. We are here to help you understand your pain that may have confused you for some time now. Our pain program goals are to reduce/eliminate pain; to improve function and/or activity level; to decrease/eliminate reliance on pills; and to decrease dependence on the health care system to control your primary pain complaint. The pain that you have been suffering from may be very unique to you but we may be able to find the cause of it through our algorithmic approach utilizing cutting-edge diagnostic techniques, and eventually find therapeutic modalities to provide satisfactory relief of pain or at least to manage it so you can return to a productive life as soon as medically possible. For some patients entering our program, at least a 50% pain reduction is our goal while for others even less than 50% reduction may be all that is possible and for some we may not be able to help at all. Your first appointment is usually for evaluation purposes. Thus, please bring your most recent medical records pertaining to your pain problem, MRI, CT scan or x-ray films and reports along with the finished medical questionnaire which is included in this welcome package. A diagnosis of your pain condition will be made and a plan of treatment will be prescribed. If you are in agreement with the plan appropriate scheduling will be done and instructions will be given. Your plan of treatment may include Nerve blocks, Minimally invasive precision diagnostic and/or therapeutic procedures, physical therapy, occupational therapy, medication, TENS unit, psychological evaluation, and further x-rays, MRI or lab work. Referrals to other specialists may also be necessary. If prior authorization for any treatment or diagnostic test is needed, we will contact your insurance carrier. Follow-up appointment is very important function of the Pain Clinic. Therefore, you will be asked to periodically contact us regarding your progress. We may contact you to determine the progress of the treatment periodically. If you have any questions, please feel free to call us. Copies of your evaluation and some progress notes will be sent to your physician and your case manager. Payment: You will be responsible for the co-payment at the time of your clinic visit whenever you make clinic visit. For every visit you must bring your insurance card and driver ’s license. Clear Water Pain Clinic accept BWC, Medicare, Medicaid, United Health, Aetna, Anthem, Emerald Health, Blue Cross HMO, QualChoice, and other private insurers. Your referring physician or primary physician should provide our office with a referral prior to your first visit if your insurance requires it. If you decide not to participate in the pain program outlined for you, please contact our office so we can notify your physician of your decision.
